Profile could not be loaded
This is the most common way I find this issue. There is almost always just one account on(active), on the computer.

I have not been able to restore profiles like the one pictured yet. A work around though is to inject a new account from a PE. (Password renew 1.1 beta http://www.kood.org/windows-password-renew-11/)

Brink that one is really easy to fix. I'll point out more detail later when I get a min.
